Terms and Conditions


Definitions

Terms used in these regulations, written with a capital letter, have the meaning given below:

  • Regulations – these regulations, constituting the content of the Agreement for participation in the Conference and additional Workshops.
  • Agreement – an agreement for the participation of the Participant in the Conference or additional Trainings, concluded between the Conference Organizer and the Participant in accordance with the provisions of the Regulations.
  • Conference – Amputation Forum “Pre-rehabilitation”, taking place on October 24-25, 2025, at Q Hotel Plus Kraków, ul. Wygrana 6, 30-311 Kraków
  • Conference Organizer – FUNDACJA CENTRUM REHABILITACJI ZNOWU W BIEGU based in Kraków, 30-408 Kraków, ul. Odrzańska 13/2, NIP 6793175570, REGON 381525090, entered into the register of associations of the National Court Register maintained by the District Court for Kraków-Śródmieście in Kraków, 11th Commercial Division of the National Court Register under number KRS 0000749510.
  • Participant – participant of the Conference or additional Workshops.
  • Sending Entity – an entity concluding an Agreement with the Conference Organizer for the participation of the Participant in the Conference or Workshops.
  • Training – a part of the Conference in which participation is possible after additional registration and payment, on the terms described in sec. III.10-11 of the Regulations. A list of available Trainings is available at www.forumamputacji.pl.
  • Conference Office – an office opened during the Conference at its venue.
  • GDPR – Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation).
  • Consumer – a natural person who enters into a legal transaction with a business entity not directly related to their business or professional activity.


I. Conference Program

The projected program outline of the Conference is available at www.forumamputacji.pl. The full Conference Program will be published by September 10, 2025.


II. Conclusion of Agreement – Registration for Participation in the Conference

  1. The party to the Agreement for participation in the Conference or Workshop may be: the Participant or the Sending Entity.
  2. If the Agreement is concluded by the Sending Entity, it acts on its own behalf, but for the benefit of the Participants indicated during registration. The Sending Entity is obliged, in particular, to pay the conference fees or, depending on the registration scope, the Training fees for all registered Participants, and is also responsible for ensuring that the Participants comply with the Regulations. The Sending Entity undertakes to familiarize registered Participants with the Regulations and any other arrangements.
  3. The Agreement (registration) is concluded by completing the online form via the online registration system available at www.forumamputacji.pl.
  4. Subject to sec. 5 below, the registration deadline is October 10, 2025.
  5. In the event that participation slots are filled before the date specified in sec. 4 above, the Organizer has the right to suspend further registration and close the participant list early.
  6. After the deadline specified in sec. 4 above, participation may only be registered on-site at the Conference Office, depending on availability, unless the Conference Organizer individually agrees to registration in another form. Consent may be given, in particular, by confirming acceptance of the registration.
  7. If the Conference Organizer accepts registration submitted after the deadline referred to in sec. 4 above, it does not guarantee the full package of materials.
  8. Participation in the Training is possible after making the payment specified in sec. IV of the Regulations.
  9. Registration for participation in the Conference and Workshop made in a manner inconsistent with the Regulations may not be accepted.
  10. For educational reasons, the number of places in the Training (described at www.forumamputacji.pl) is limited. The Participant will be able to attend the Training (the fee for participation is provided at www.forumamputacji.pl) only after selecting the appropriate option in the online form, subject to availability, and after completing all formalities. Qualification for participation depends on the order of payments received in the Conference Organizer’s bank account. If the payment for a specific Participant is received after all available slots are taken, the Organizer will refund the payment within 7 days to the bank account from which it was made and inform the Participant or Sending Entity using the contact details provided during registration.


III. Fees – Amount and Scope

  1. Participation in the Conference is free of charge. The fee for participating in Trainings is available at www.forumamputacji.pl.
  2. The fee for Training participation for Participants INCLUDES:
    • participation in the selected workshop,
    • a package of conference materials,
    • a certificate of attendance,
    • coffee break and lunch.
  3. Payment can be made via bank transfer based on a proforma invoice sent to the email address provided in the registration form.
  4. In case of bank transfer, the Participant or Sending Entity must make the payment within 3 days of concluding the Agreement (registering via the online system).
  5. If no payment or an insufficient payment is made, the Participant or Sending Entity will be requested to pay the remaining amount. If not completed, the Organizer may cancel the registration by October 10, 2025. If a payment was made, it will be refunded to the originating bank account, minus 30% of the paid amount. For Consumers, refunds will be processed in accordance with applicable Polish law.
  6. Participation in the Conference and Workshops is possible only after payment is made.
  7. The VAT invoice details provided during registration are final. The VAT invoice will be issued according to the submitted data.


IV. Withdrawal from Participation in the Conference or Trainings and Change of the Paying Entity

  1. The Participant or the Sending Entity has the right to withdraw from the Agreement without providing a reason under the terms described below.
  2. In the case of a withdrawal statement received by the Organizer:
    • a) by October 10, 2025, the Participant or the Sending Entity will receive a refund of 70% of the Workshop fee,
    • b) after October 10, 2025, the Participant or the Sending Entity will not be entitled to a refund of any part of the conference or Workshop fee.

    The provisions of points a) and b) do not apply to Consumers, for whom refunds will be settled based on the general principles of the law in force in Poland, subject to item 3 below.

  3. Apart from the right to withdraw referred to in item 2 above, the termination of the Agreement by the Participant or the Sending Entity is excluded to the extent permitted by applicable law.
  4. The withdrawal statement should be sent in the following form:
    • by email to: centrum@znowuwbiegu.pl (the resignation email should be sent from the email address provided during registration),
    • by mail to the Organizer’s address (the date of receipt by the Organizer is taken into account; this does not apply to Consumers, for whom the date of dispatch is decisive).
  5. Refunds will be made to the bank account indicated by the Participant or the Sending Entity who made the payment.
  6. In the event of a change in the entity financing the Participant’s attendance at the Conference, i.e., a refund to the Participant of a fee covered by a third party, the Organizer will refund the Participant the paid amount minus 30%. The refund will be made after the Organizer receives the payment from the new paying entity. This provision does not apply to Consumers, who are subject to the general principles of Polish law regarding refunds.


V. Personal Data

  3. Categories of Data
    If the data is provided by a registering entity, the following data will be processed: full name, email address, phone number, place of employment.
  4. Purposes and Legal Bases for Processing
    • performance of the contract related to Conference participation (Art. 6(1)(b) GDPR),
    • accounting and banking obligations (Art. 6(1)(c) GDPR),
    • reporting and compliance with agreements with public institutions and sponsors (Art. 6(1)(c) GDPR),
    • marketing and information on events, based on consent (Art. 6(1)(a) GDPR),
    • establishing or defending claims – legitimate interest (Art. 6(1)(f) GDPR).

    Participation in the Conference implies consent to the use of image (Art. 6(1)(a) GDPR) in promotional materials and communication channels of the Organizer, without financial claims.

  5. Voluntary Data Submission
    Providing data may be voluntary but necessary to fulfill the above purposes. Failure to provide data may result in the inability to participate.
  6. Data Recipients
    Data may be shared with entities processing data on behalf of the Controller (e.g., IT, accounting, legal advisors), and upon marketing consent – with cooperating partners.
  7. Data Retention
    • based on contract – until expiration of claims and as required by tax regulations,
    • based on consent – until the consent is withdrawn.
  8. Data Subject Rights
    The following rights are granted:
    • access to personal data and copy thereof,
    • rectification, erasure, restriction of processing,
    • data portability,
    • objection to processing,
    • withdrawal of consent at any time.

    Requests should be addressed to the Data Controller.

  9. Complaint to Supervisory Authority
    You have the right to lodge a complaint with the President of the Personal Data Protection Office if the data is processed unlawfully.
  10. Automated Decision-Making
    Personal data will not be processed in an automated way, including profiling.
  11. Transfer Outside the EEA
    Data will not be transferred to international organizations, but may be transferred to third countries (e.g., the USA) through tools provided by companies based outside the EU.


VI. Liability

  1. The Participant agrees to comply with fire and health and safety regulations applicable at the Conference and Workshop venues.
  2. The Participant is responsible for their belongings at the Conference and Workshop venues.
  3. The Participant will be liable for any damage caused during the Conference or Workshops, especially to the Organizer, other Participants, or the facility provider.
  4. The Organizer is not liable for lost profits. This limitation does not apply to Consumers, to whom general provisions of Polish law apply.


VII. Final Provisions

  1. Any disputes will be resolved by the court competent for the Organizer’s registered office.

  3. Complaints must be described in detail. They will be processed without delay, no later than 10 working days after receiving all necessary information.
